Services Provided

Provider agrees to offer [list specific services, e.g., virtual consultations, specialist services] through True Med’s platform.

Provider will maintain the required licenses and certifications for the services provided.

Compensation

True Med will charge patients a fee i.e. $50 per consultation.

True Med will retain 20% (i.e. $10) of the consultation fee. The remaining 80% (i.e. $40) will be remitted to the Provider.

Payments will be processed bi-weekly via [Payment Method].

Responsibilities

Provider: Ensure timely and professional delivery of services, maintain up-to-date licenses, and comply with healthcare regulations.

True Med: Provide the platform, manage patient interactions, and facilitate payments.

Confidentiality and Data Protection

Both parties agree to keep patient data confidential and comply with applicable data protection laws (e.g., HIPAA, GDPR).

Termination

Either party may terminate this Agreement with 30 days' written notice. Immediate termination is allowed in cases of gross negligence or breach of contract.

Dispute Resolution
